Case studies are often used in patient care simulations for online medical education, where learners work through realistic clinical scenarios and apply their knowledge to clinical decision-making. They can also support learning across many other forms of medical education.

This blog covers what medical case studies are, how they differ from clinical case reports, why they are useful in medical education, and how medical writers can develop engaging, accurate, and educational case studies.

## **What Is a Medical Case Study?**

A medical case study generally presents a clinical scenario that gives health care professionals the opportunity to apply their knowledge and skills to patient care decisions. Case studies create realistic learning challenges that reflect situations physicians, nurses, pharmacists, and other health care professionals may encounter in clinical practice.

[MedCaseWriter](http://medcasewriter.com), a resource developed by Scott Kober, described the value of case studies this way: “A good case study brings clinicians into the virtual classroom, allowing them to make mistakes and play/replay scenarios in a protected environment that they can then incorporate into their day-to-day practice.”

### **How Does a Medical Case Study Differ from a Clinical Case Report?**

 A clinical case study or case report [describes and discusses the diagnosis, treatment, or outcome of an actual patient](https://blog.amwa.org/how-to-write-your-first-clinical-case-report) or a small number of patients, often for publication in a medical journal. Tools such as the [CARE (CAse REport) guidelines](https://link.springer.com/article/10.1186/1752-1947-7-223) provide guidance for writing complete and transparent clinical case reports.

In contrast, case studies developed for medical education are designed primarily as learning tools and may be based on real, adapted, or hypothetical clinical scenarios.

## **Why Case Studies Matter**

Case studies are valuable educational tools because they allow learners to apply their knowledge to realistic clinical situations, helping [bridge the gap between theory and practice](https://pmc.ncbi.nlm.nih.gov/articles/PMC5736264/#section3-JMECD.S20377) while encouraging clinical reasoning and critical thinking.

According to medical writer Donald Harting, MA, MS, ELS, CHCP*,* “They help people to integrate previous experiences and create a sense of immediacy and authenticity during the learning process.”

## **Elements of a Medical Case Study**

An engaging case study draws learners into the scenario and helps them connect with the content. “You need to figure out a way to present the information in a way that resonates with the learners,” Harting wrote. “You are essentially writing a short story, so think about the cast of characters and the plot.”

The story within a medical case study typically unfolds in stages, guiding learners through a clinical scenario as it develops. A common progression might include:

- **Patient presentation:** Introduce the patient and provide relevant information, such as medical history, symptoms, and clinical findings.
- **Clinical challenge:** Present the problem the learner needs to address, such as determining a diagnosis, selecting appropriate tests, or choosing a treatment or management approach. Ask the learner to determine the next appropriate step, sometimes through a multiple-choice question.
- **Case progression:** Provide additional information as the clinical situation develops, including the consequences or outcomes associated with the course of action.
- **Additional challenges and decisions:** Continue the scenario with new information or changes in the patient’s condition that require further assessment or decision-making.
- **Outcome and discussion:** Conclude with the patient outcome and, when appropriate, explain the clinical reasoning or evidence supporting the preferred approach.

## Tips for Writing Effective Medical Case Studies

Writing an effective and engaging story for a case study requires careful preparation, thorough research, appropriate language, and attention to accuracy.

### **Prepare by Posing Questions**

When developing a case study, the [Centers for Disease Control and Prevention (CDC) suggests considering questions](https://www.cdc.gov/nceh/clearwriting/case_study_guide.html) such as:

- Who is the audience?
- What are your goals?
- How much knowledge does the audience have?
- What is the format, such as online, a handout, or part of a journal publication?

 In addition,[Kober emphasizes](https://www.medcasewriter.com/why-case-studies) that successful case studies answer the following questions about the scenarios they portray:

- What is the patient’s medical, family, demographic, and socioeconomic background?
- What is the problem the practitioners are trying to solve?
- What steps can be taken to manage or treat the patient?
- Why is one choice better than another?
- What is the final outcome?

### **Research the Topic**

Thorough research is essential for developing an accurate and realistic medical case study. Depending on the project, useful research sources may include:

- **Published case reports** in the same therapeutic area to understand common clinical issues and how clinical information is presented.

- **Current clinical practice guidelines** to help ensure the case reflects evidence-based recommendations for diagnosis, treatment, and patient management.

- **Existing case-based education materials**, such as other CME case studies, to get a sense of format, pacing, and question style.

- **Clinician perspectives**, such as interviews, videos, or articles that discuss clinical reasoning and approaches to patient care.

Medical writers may also work with a subject matter expert (SME) throughout development to ensure the case is clinically accurate and aligned with the activity’s learning objectives.

### **Choose Appropriate Language**

The language you use should reflect the audience, purpose, and voices represented in the case study. For clinician-facing content, you might use the writing style, vocabulary, and common abbreviations found in electronic medical records (EMRs), as recommended by Allison Armagan, PharmD, director of clinical strategy for Medscape.

According to Armagan, this vocabulary is familiar to many clinicians, which can make scenarios feel more realistic. To study an example of EMR language, Armagan recommends reviewing a copy of your own medical record.

If the case includes dialogue from a hypothetical patient, the patient’s words should also sound natural and realistic. Publicly available patient blogs or online communities can help writers understand how people describe their conditions, symptoms, and experiences in their own words.

### **Consider Visual Elements**

Visual elements such as photographs, X-rays, or excerpts from lab reports can make case studies more engaging and add realism to the clinical scenario.

Depending on the project, medical writers may collaborate with graphic designers to develop illustrations or other visuals. When using previously published material, be sure to obtain permission from the copyright holder when required. For public domain materials, check whether attribution or other credit is appropriate.

### **Edit, Proofread, and Check Facts**

As with any writing project, you’ll need to edit, or have an editor review, the piece for grammar, tone, style, and coherence.

Accuracy is equally important. Drug dosing, for example, must be correct and appropriate, Kober says. Errors can undermine credibility, and even seemingly minor mistakes can have consequences. A stray period or extra space, for instance, can cause a program to malfunction, according to Jill Herr, MD, an AMWA member who writes case studies for Syandus Inc.

When an SME is involved, they may also review the completed case to confirm clinical accuracy, ensure the content is appropriate for the intended audience, and verify that the scenario reflects real-world practice and the activity’s learning objectives.

## **Writing Effective Multiple-Choice Questions**

Writing effective multiple-choice questions requires more than identifying one correct answer. Each question should reinforce the purpose of the case and give learners a meaningful opportunity to apply what they have learned.

Consider the following when developing questions:

- **Align each question with a learning objective.** The question should assess knowledge or decision-making that directly supports what learners are expected to take away from the activity.

- **Make the case necessary to answer the question.** Learners should need information from the clinical scenario to identify the correct answer. If the question can be answered without reading the case, it may be testing general knowledge rather than making full use of the case-study format.

- **Write plausible distractors.** Incorrect answer choices should reflect realistic clinical misconceptions, practice gaps, or alternative decisions rather than obviously incorrect options. Published literature or a needs assessment, when available, can help identify these.

- **Keep the options clinically accurate.** Correct answers should reflect real-world practice and should not, for example, involve a dosing schedule or administration route that clinicians would not use.

- **Review previous questions when possible.** Examples from the organization or activity can help you become familiar with its approach, difficulty level, and style.

## **Putting Your Case Study Writing Skills Into Practice**

Well-designed clinical scenarios can help health care professionals apply what they learn to realistic patient care situations and support stronger clinical practice.
